St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive focused on hospitals in the Neenah area. The role develops and maintains hospital referral relationships and educates staff on hospice care while supporting timely referral coordination.

You will collaborate with clinical partners and coordinate marketing activities to drive patient referrals and business growth. The ideal candidate has 3+ years in a sales role, excellent communication and negotiation skills, and the

Qualifications

3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
Associate's degree in Marketing/Business/Healthcare preferred.
Ability to position services to customers and clients; strong communication and negotiation skills.
Autonomy, organization, assertiveness, flexibility and teamwork in performing job responsibilities.

Responsabilités

Develop and maintain positive relationships with hospital referral sources and designated communities.
Provide education on hospice care to hospitals and referral sources.
Assist intake to ensure timely and accurate referral coordination.
Coordinate daily sales and marketing operations, including marketing initiatives and events.
Document all sales activity in the CRM per organizational requirements.
Monitor and influence market perceptions of St. Croix Hospice as a high-quality provider.
