Job summary

St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive for Hospitals to develop relationships with hospital referral sources and educate on hospice care.

You will coordinate admissions, support intake, and advance marketing strategies while partnering with clinical teams to grow patient referrals. The role requires building strong relationships, meeting sales targets, and leveraging CRM tools to track activities and outcomes.

Qualifications

• 3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
• Ability to position services to, and deal tactfully with, customers and clients
• Demonstrates good communications skills, negotiation skills, and public relations skills.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider.
• Collaborate with clinical partners to drive patient referrals and business growth.
• Document all sales activity in the CRM per requirements.
• Mentor and support Care Transition Coordinators in territory management.
• Assist with orienting new marketing staff and events.
