St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive focused on hospitals in Wisconsin. You will develop relationships with referral sources, provide hospice education, and assist intake to ensure timely referrals.

The role emphasizes collaboration with clinical partners to drive patient referrals and business growth. The position requires an associate’s degree in marketing/healthcare and 3+ years in sales, with strong communication and negotiation skills.

Formación

• 3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
• Ability to position services to customers and clients with tact.
• Excellent communication, negotiation, and public relations skills.

Responsabilidades

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including implementation of marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider of services.
• Collaborate with clinical partners to drive patient referrals and business growth.
• Document all sales activity in the CRM per organizational requirements.
• Employ marketing initiatives to achieve budgetary sales volume projections.
• Monitor markets, payer groups, and referral sources.
