Agrace is seeking a Patient Care Coordinator in Madison, WI. This full-time remote-enabled role supports Homecare and Resource visit scheduling and staff coordination, ensuring timely patient care.

Qualifications

• High school diploma or GED required.
• Previous scheduling experience preferred.
• Ability to work weekend rotation.
• Intermediate Microsoft Office and EHR skills.
• Ability to maintain confidentiality about patients, families, staff, and donors.

Responsabilités

• Schedule Homecare and Resource visits for patients.
• Coordinate staffing to cover patient needs and absences.
• Assign visits to clinical staff ensuring consistency and using scheduling best practices.
• Manage On Call schedules for Home Care and Resource staff.
• Provide responsive customer service and timely communications.
