Job summary

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking a Homecare Manager in Eau Claire, WI. This full-time role focuses on leading the Homecare team, coordinating care through an interdisciplinary approach, and ensuring high-quality, compliant, patient-centered services.

Responsibilities include staffing, on-call leadership, staff development, and collaboration with regional leadership to optimize census, care outcomes, and reimbursement practices.

Qualifications

• Bachelor’s degree with clinical background.
• RN license in Wisconsin preferred.
• 5–7 years in clinical leadership or healthcare administration, hospice experience preferred.

Responsibilities

• Oversee daily homecare operations and staffing to meet census and patient needs.
• Guide coordination of care using interdisciplinary teams (IDT).
• Monitor quality, safety, regulatory compliance, and patient/family satisfaction.
• Lead recruitment, coaching, and staff development.
• Collaborate with volunteers and leadership for 24/7 on-call support.
• Manage financial performance and care delivery efficiency.
