Job summary

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking a dedicated CNA for homecare in Madison, WI. You will support patients with daily living activities, mobility, hygiene, meals, and essential comfort measures while following the care plan and documenting changes in the health record.

This role includes weekend and holiday rotations, starting pay $18.50/hour with a $3,000 sign-on bonus, and a strong benefits package.

Qualifications

• Must have CNA certification and a valid state license.
• High school diploma or GED required or equivalent
• Hospice or home health experience preferred

Responsibilities

• Assist patients with activities of daily living.
• Assist with ambulation and positioning.
• Help with bathroom functions and incontinence care.
• Assist with bathing, dressing, oral care, grooming.
• Assist with eating and drinking and monitor intake.
• Take vital signs and report changes to the team.
• Document care in the electronic health record.
• Read and follow the plan of care and reinforce education.

If you are a skilled CNA who values meaningful, patient-centered care, this is your opportunity to make a difference!How You’ll Make a Difference (Essential Functions)The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.Key ResponsibilitiesAssist Patients with Activities of Daily LivingAssist with ambulation, changing position, and moving to/from sitting and standing.Help with bathroom functions like toileting and incontinence care.Help bathe, shower, dress, provide oral care, and groom.Assist with eating and drinking.Perform post mortem care.Obtain vital signs. Assist with treatments.Anticipate and respond to patient needs.Provide homemaker assistancePerform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.Order and deliver patient supplies.Help prepare simple meals.CommunicationRead, understand, and follow the plan of care.
