Job summary

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, seeks a Resource Registered Nurse in Madison, WI. This 0.9 FTE role covers homecare teams, triages patient needs, and supports planning of care across disciplines.

The Resource RN moves across teams as needed, with day shift hours, rotating weekends every 3rd, and holiday rotations. Sign-on bonus of $6,000!

Qualifications

• ADN required; BSN preferred
• Active RN license in Wisconsin
• 1 year nursing experience preferred
• Valid driver’s license and access to reliable vehicle
• Must be at least 18 years old
• Basic Microsoft Office skills
• Intermediate EHR skills

Responsibilities

• Assess patients and families and document plan of care.
• Provide nursing interventions, pain management, and education.
• Develop and revise individualized plan of care with IDT.
• Oversee CNA/LPN activities and delegation of visits.
• Participate in meetings and precept new staff.
