Job summary

Agrace, a leading hospice and palliative care nonprofit in Wisconsin, seeks a Social Worker on the Resource Team to assess patients and families, advocate for goals, and coordinate care across homecare teams. The role involves on-call rotations and collaboration with an interdisciplinary team to ensure patient-centered care in a 40-hour week.

The ideal candidate holds a BSW with MSW preferred, an active WI social work license, and strong communication and care coordination skills.

Qualifications

• Bachelor’s in Social Work (BSW) is required.
• Master’s in Social Work (MSW) preferred.
• Current, unrestricted Social Work license in state of employment.

Responsibilities

• Conduct initial and ongoing patient/family assessments.
• Coordinate transition planning with patient/family and IDT.
• Develop and revise Plan of Care in the EHR with goals and interventions.
• Provide education and resources to patients and families and facilitate bereavement support.
