Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. You will assess patients, develop and revise care plans, and administer prescribed treatments while supporting families.

The role emphasizes coordination with physicians and interdisciplinary teams, on-call duties, and ongoing professional development in a hospice setting. A valid RN license and CPR certification are required.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing.
• 1–2 years of recent nursing experience preferred, in public health/home care/hospice is a plus.
• supervision of hospice aides not required.
• Contingent upon references and background checks.
• Excellent observation, communication, and math skills.
• Physical ability to lift/move patients and equipment; valid driver with insured car.
• CPR certification required (hands-on verification).

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop plans of care and revise as patient needs change.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Deliver health care instructions and support self-care.
• Serve as Case Manager when assigned and coordinate patient care.
• Document clinical notes accurately and timely.
• Coordinate with physicians and community health teams.
• Participate in on-call duties and ensure equipment and services are available.
• Contribute to quality improvement and interdisciplinary team activities.
