Qualifications

• Active RN license in good standing
• Minimum 1 year of professional nursing experience within the last 3 years or 2 years with BSN, in public health/home care/hospice preferred
• Non-management; may supervise hospice aides as needed
• Criminal background checks required and references may be requested
• CPR certification required and maintained

Responsibilities

• Assess patient and family needs and develop a care plan
• Provide nursing care and administer prescribed medications
• Coordinate patient care as case manager with the interdisciplinary team
• Document patient condition and care provided
• Educate patient and family on self-care techniques and disease management
• Participate in on-call duties and ensure equipment and services are available
