Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize, and direct hospice care in the community. The RN will work with patients and families in home settings, coordinating care with the interdisciplinary team to meet individualized hospice needs.

Qualifications

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status and update the plan of care as needed.
• Initiate and revise the plan of care as patient needs change and coordinate with the care team.
• Provide patient/family education and support for self-care and comfort.
• Coordinate care as Case Manager and ensure continuity across team members.
• Maintain accurate clinical documentation and timely communication with physicians.
