Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize and direct hospice care for patients in home and community settings. The RN will use the nursing process to assess needs, develop plans, and coordinate care with families and the interdisciplinary team.

The role requires a current RN license in good standing, strong communication skills, and the ability to lift and transfer patients. CPR certification is required, and prior hospice/home care experience is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing.
• Minimum 1 year of professional nursing experience within last 3 years; or BSN with 2 years' experience in health areas.
• Experience preferred in public health, home care, or hospice.
• Must be able to lift and transfer patients; physical endurance required.
• CPR certification required.

Responsibilities

• Complete initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Administer meds and treatments as prescribed.
• Counsel patient and family in nursing and related needs.
• Maintain accurate clinical notes and report changes to physician.
• Coordinate care with the interdisciplinary team and participate in meetings.
• Participate in on-call duties and ensure equipment/ services are available.
• Supervise hospice aides and delegate tasks as needed.
• Maintain safety and a conducive environment for patients.
