Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize, and direct hospice care in home settings. The role emphasizes community health education and family-centered care, built from community resources to meet patient needs.

The RN will conduct comprehensive assessments, develop and revise care plans, coordinate with physicians, and mentor aides while ensuring safety and quality across the continuum of care.

Qualifications

• Must have active RN license in good standing.
• Nursing degree from an accredited program or equivalent.
• Excellent observation, verbal and written communication skills.
• Ability to implement nursing process and care plans.

Responsibilities

• Complete initial, ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Educate patients and families on self-care techniques.
• Maintain safe environment and coordinate care with the interdisciplinary team.
• Participate in on-call duties as defined by policy.
• Supervise hospice aides and coordinate equipment and services.
• Maintain clinical notes and documentation as required by regulation.
