Qualifications

• Graduate of an accredited nursing program and RN licensed in good standing.
• 1 year of nursing experience in the last 3 years, or a qualifying nursing degree with relevant public health/home care experience.
• CPR certification required.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Document clinical notes and report changes to physicians.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise hospice aides and contribute to QA efforts.
