Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize, and direct hospice care in community/home settings. The RN will lead comprehensive patient/family assessments, coordinate care, and provide counseling and instruction on health management.

The role requires licensure in good standing, CPR certification, and a driver’s license. Prior public health/home care/hospice experience is preferred. On-call duties may be required.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• One year of experience as a professional nurse within the last 3 years or a BSN with two years’ experience (preferably in public health, home care, or hospice).
• Must be a licensed driver with a valid automobile insurance.
• CPR certification required (hands-on verification preferred).

Responsibilities

• Complete initial and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care via nursing process.
• Develop and revise plan of care with patient and family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing needs and self-care techniques.
• Assist with activities of daily living and care coordination as Case Manager when assigned.
• Communicate with physicians and community health professionals to coordinate care.
