Job summary

Affinity Hospice Management LLC is seeking an experienced registered nurse to plan, organize and direct hospice care in home and community settings. You will assess patients, develop care plans, administer treatments, and educate families while coordinating with physicians and the interdisciplinary team.

The role requires an RN with current license in good standing, 1 year of recent nursing experience (or BSN with 2 years in public health/home care), and strong communication skills.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN with good standing to practice in the state.
• 1 year of nursing experience in last 3 years; or BSN with 2 years' related experience.
• Supervising hospice aides may be required.
• Clear references and background checks required.
• Strong observation, communication and math abilities.
• Able to lift/move patients and supplies; mobility skills.
• Valid driving license with insured vehicle.

Responsibilities

• Complete initial and ongoing patient and family assessments for hospice needs.
• Provide professional nursing care following the nursing process.
• Develop and revise plans of care with the team and family involvement.
• Administer medications and therapies as prescribed.
• Counsel patients and families on nursing needs and self-care.
• Document clinical notes and care changes accurately.
• Coordinate with physicians and community health partners.
• Report compliance issues and maintain safety standards.
• Participate in interdisciplinary team meetings and education.
• Participate in on-call duties and ensure equipment availability.
