Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse with hospice experience to plan, organize and direct patient care in home and community settings. The RN will assess, plan, implement, and evaluate care with emphasis on family involvement and education.

The role includes coordinating with an interdisciplinary team, providing medications and treatments as prescribed, and supporting patients and families through end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing.
• 1 year minimum of professional nursing experience within the last 3 years, or a BSN with 2 years' experience in public health, home care, or hospice.
• Supervisory responsibilities of hospice aides not required.
• Criminal background check and references required after offer.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and develop/evaluate care plans.
• Collaborate with physician to coordinate orders; update plans as patient status changes.
• Counsel patient and family; give health instructions and support self-care.
• Participate in on-call duties and quality/performance improvement activities.
