Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care in home and community settings. The role emphasizes community health education and collaboration with the interdisciplinary team to meet patient and family needs.

The nurse will conduct comprehensive assessments, develop and revise plans of care, administer medications, provide patient/family education, and participate in on-call rotations. A valid RN license and CPR certification are required.

Formación

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing to practice.
• Minimum of 1 year of professional nursing experience within the last 3 years, or a BSN with 2 years' experience (public health/home hospice preferred).
• CPR certification required (hands-on verification).
• Must possess a valid driver's license with insured vehicle.

Responsabilidades

• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Develops and initiates plan of care with input from patient and family; revises as needed.
• Provides professional nursing care using the nursing process and administers medications/treatments as prescribed.
• Counsels patient and family on nursing and related needs; conducts health education as appropriate.
• Acts as case manager for assigned caseload; participates in on-call duties and interdisciplinary team meetings.
