Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education and strong nursing practice to meet the needs of individuals and families.

You will conduct comprehensive assessments, develop and revise care plans, administer medications, counsel patients and families, and coordinate with an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program.
• Licensed RN in good standing.
• Experience in home health, hospice, or public health preferred.
• Current CPR certification.
• Ability to lift and transfer patients; valid driving license.
• Excellent observation and communication skills.

Responsibilities

• Complete comprehensive initial and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of nursing process.
• Develop and revise plans of care with patient/family involvement.
• Administer medications and treatments as prescribed by the physician.
• Counsel the patient and family in meeting nursing and related needs.
• Coordinate care as Case Manager with Clinical Supervisor and assigned caseload.
• Complete and submit accurate clinical notes and report changes to physician.
• Attend interdisciplinary group meetings and participate in quality assurance activities.
