Job summary

Affinity Hospice Management LLC in Connecticut is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings, drawing on nursing expertise and community health experience.

The RN will conduct comprehensive patient assessments, develop care plans, coordinate with physicians and the interdisciplinary team, and educate patients and families on care options and symptom management.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• 1 year of professional nursing experience within the last 3 years, or a BSN with 2 years’ experience in public health/home care/hospice preferred.
• Excellent observation, verbal and written communication skills; problem solving and math skills required.
• Must possess CPR certification and a valid driver’s license with reliable vehicle.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Develops and revises the plan of care based on patient status and nursing diagnoses.
• Delivers professional nursing care using the nursing process and administers medications as prescribed.
• Counsels patient and family and provides health care instructions as appropriate.
• Coordinates with physician and interdisciplinary team to ensure continuity of care and quality.
