Job summary

Affinity Health Management in New Haven, CT is seeking a registered nurse to plan, organize and direct hospice care, drawing on community health experience to serve patients at home and in the community.

Responsibilities include comprehensive patient and family assessments, developing and revising the plan of care, administering prescribed medications, counseling patients and families, and coordinating care as a case manager when assigned.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• RN with 1 year of professional nurse experience within the last 3 years; or a baccalaureate degree in nursing with 2 years' experience (preferred in public health/home care/hospice).
• Management experience not required; responsible for supervising hospice aides.
• Contingent upon satisfactory references and criminal background checks.
• Excellent observation, verbal and written communication skills, problem solving, and basic math; nursing skills per competency checklist.
• Prolonged walking/standing; able to lift, position and transfer patients and equipment.
• Licensed driver with insured automobile in good working order.
• CPR certification (hands-on, not just online) required.

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs; performs complete physical assessment and histories.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Develops and initiates a care plan, revises it as patient status changes, and involves patient and family in planning.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs; provides health care instructions.
• Acts as Case Manager when assigned by the Clinical Supervisor and coordinates patient care.
