Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care; initiate and adjust care as patient needs change.
• Administer medications and treatments as prescribed.
• Counsel patient and family; provide health instructions and self-care teaching.
• Act as Case Manager for assigned caseload; coordinate patient care.
