Qualifications

• Graduate of NLN-accredited school of nursing.
• Current licensure in State.
• Minimum 1 year of professional nursing in last 3 years; or BSN with RN license.
• 2 years’ experience in public health, home care, or hospice preferred.

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Assess and re-evaluate patient status; revise plans of care as needed.
• Initiate and modify the plan of care as patient needs change.
• Use health data to determine nursing diagnoses and establish goals.
• Develop and implement care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Provide health care instructions and assist with activities of daily living.
• Coordinate patient care as Case Manager when assigned by supervisor.
