Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize, and direct hospice care in home and community settings. The RN leverages community resources to tailor services for patients and families, coordinating care across disciplines.

The role emphasizes assessment, planning, and patient-family education, with responsibilities for care planning, medication administration, and on-call support as part of a collaborative hospice team.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice.
• Minimum of 1 year of professional nursing experience within the last 3 years; or a bachelor’s degree in nursing with 2 years’ experience (preferred).
• Management experience not required; responsible for supervising hospice aides.
• Offer contingent upon references and background checks.
• Excellent observation, verbal and written communication, problem solving, basic math skills; nursing skills per competency checklist.
• Ability to walk/stand for long periods; lift/position patients and equipment.
• Must be a licensed driver with insured vehicle in good working order.
• CPR certification is required (hands-on).

Responsibilities

• Completes initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care; regularly reevaluates patient and family needs.
• Initiates plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis and risk factors.
• Develops a care plan with goals based on nursing diagnosis and palliative actions.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions per assessment and plan.
• Assists activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager when assigned and coordinates patient care.
