Job summary

Affinity Health Management is seeking a registered nurse to plan, organize, and direct hospice care in home and community settings. The RN will build on community resources to meet patient and family needs through comprehensive assessments and care planning.

The role emphasizes nursing process, patient education, care coordination, and collaboration with the interdisciplinary team. A valid RN license and CPR are required, with on-call duties and frequent mobility.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice.
• Minimum of 1 year of professional nursing in the last 3 years, or a BSN with 2+ years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• References and criminal background checks required after offer.
• Excellent observation, verbal and written communication; basic math; nursing skills.
• Must be able to lift/move patients and supplies; good hearing and vision.
• Must have a valid driver's license and insured vehicle.
• CPR certification required (hands-on proof).

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and revises the plan of care; regularly re-evaluates patient and family needs.
• Initiates and revises plans of care as patient status changes.
• Develops care plans based on nursing diagnosis and palliative actions.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health instructions and supports self-care and comfort care.
• Acts as Case Manager for assigned caseload; coordinates patient care.
• Participates in on-call duties and coordinates equipment and services.
• Supervises ancillary personnel and maintains professional knowledge.
