Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will conduct comprehensive assessments, develop a patient-centered care plan, and coordinate with an interdisciplinary team to ensure compassionate end-of-life care for patients and families.

The role emphasizes nursing assessment, plan of care development, patient/family education, and case management responsibilities, with required driving and CPR

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing.
• Minimum 1 year nursing experience in the last 3 years; hospice/home health/public health preferred.
• Excellent observation and communication skills; problem solving abilities.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care following the nursing process; administer medications as prescribed.
• Develop and revise the plan of care with patient and family involvement; coordinate with interdisciplinary team.
• Ensure safety and quality of care, document accurately, and educate patient/family on self-care.
