This role requires a Virginia Certified Nurse Aide license, English proficiency, and the ability to document care in hospice software. Hours typically align with patient needs and RN plans, with ongoing in-service training.

Qualifikationen

• Must be licensed by the Virginia Department of Health Professions as a Certified Nurse Aide.
• Ability to communicate effectively with patients, families and the care team.
• Proficient in documenting patient care in hospice software.

Aufgaben

• Assist with personal hygiene, feeding and mobility per the RN care plan (7:00–3:00 shift).
• Support education for patient caregivers about end-of-life symptoms and concerns.
• Document care provided and patient responses; report changes to RN.
• Collaborate with interdisciplinary team to implement patient and family plans.
