Qualifikationen

• Licensed registered nurse in Virginia.
• Minimum 5 years of nursing experience, hospice/acute/long-term care preferred.
• CPR certification required; ability to document care accurately.
• Valid driver's license and reliable vehicle; travel to home/community settings.
• Proficient with Word and clinical documentation systems.
• Ability to work independently and communicate clearly with patients and families.

Aufgaben

• Assess needs and provide support by phone or in person per hospice standards.
• Deliver care to promote comfortable, dignified end-of-life outcomes.
• Collect data on physical, psychological, social, and spiritual factors.
• Evaluate level of care needs with physicians and on-call staff.
• Arrange for medications and supplies after hours and weekends.
• Respect beliefs and provide culturally sensitive support.
• Document nursing visits, medications, changes, and plans within 24 hours.
• Teach household members and facility staff in basic caregiving.
• Engage in goals-of-care conversations with residents and families.
• Coordinate admissions and triage referrals for urgent needs.
