Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Care Navigator in West Palm Beach. This onsite, full-time role supports admissions and operates Thursday through Sunday, 8:00 p.m.–7:30 a.m. You will guide referrals through intake and collaborate with the care team to ensure a seamless transition for patients and families.

You will provide empathetic, patient-centered service, maintain records, and work with interdisciplinary staff to uphold our mission of Full Life Care.

Qualifications

• Associate degree preferred.
• Three years of healthcare administration or customer service experience.
• Knowledge of hospice or home health programs preferred.
• Ability to handle a high call volume.
• Strong communication, organization, and computer skills.

Responsibilities

• Facilitate conversion of referrals to admissions through intake.
• Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
• Communicate effectively with interdisciplinary team to ensure timely admissions.
• Maintain accurate records and documentation per policies.
• Support a compassionate, patient-centered environment reflecting our values.
