Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse to join our Admissions Team serving Broward County. The Admissions RN guides patients and families through one of life’s most significant transitions with empathy, clarity, and dignity, coordinating care from referral to enrollment.

You will work Fri-Sun 9:00am–9:30pm, collaborate with physicians and liaisons, educate about hospice services, and advocate for the hospice philosophy within the healthcare

Qualifications

• Florida RN license required; BSN preferred.
• Hospice certification preferred.
• Basic Life Support (BLS) certification required.
• Experience in public health, home health, oncology, medical/surgical, geriatric, or hospice nursing (1 year preferred).
• Proficiency with EMR software and basic computer skills.

Responsibilities

• Evaluate patients referred for hospice services.
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care.
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice-appropriate plans of care.
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long-term care facilities, and other healthcare professionals.
• Promote and advocate for the hospice philosophy through education and relationship-building within the healthcare community.
