Job summary

Trustbridge Hospice, part of Empath Health, is seeking an Admissions Care Partner for our West Palm Beach team. This on-site, full-time role follows a Tue–Sat 8:30 a.m. to 5:00 p.m.

Qualifications

• Bachelor's degree preferred in health related or human services.
• 3–5 years of experience in customer service/referral in healthcare or community settings.
• Strong understanding of medical terminology preferred; hospice experience desirable.
• Ability to communicate physical and psychosocial needs of patients and families.
• Organized, flexible, self-motivated; communicate to all levels of a medical team.
• Able to work independently with minimum supervision and meet deadlines.

Responsibilities

• Present our hospice program and continuum of care to families/patients and facilitate admissions.
• Process referrals, calls, obtain signed consents, and complete non-nursing documentation.
• Assist admissions from Intake to the Field while upholding Trustbridge values.
• Coordinate plan of care and respond to patient needs as a problem solver.
• Articulate continuum of care and guide families to necessary services.
• Represent the organization and ensure continuity of care for patients and families.
