Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate Admissions Coordinator for our West Palm Beach site. This onsite, full-time role supports the admissions process and ensures a seamless, empathetic experience for patients and families during a meaningful transition. Work Monday through Friday, 12:00 p.m.

Qualifications

• Associate degree preferred.
• Three (3) years of experience in healthcare administration or customer service.
• Knowledge of hospice or home health programs preferred, with willingness to develop clinical understanding.
• Ability to handle a high call volume.
• Strong communication, organization, and computer skills.

Responsibilities

• Facilitate the conversion of referrals to admissions through completion of the intake process.
• Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
• Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
• Maintain accurate records and documentation in accordance with organizational policies.
• Support a compassionate, patient-centered environment that reflects the mission and values of Trustbridge Hospice.
