Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate RN to join our Admissions Team serving Broward County. Work schedule is Monday-Thursday 12pm-10:30pm. The role involves evaluating referred patients, educating families about hospice care, and coordinating personalized care plans with clinicians.

Requires Florida RN license; BSN and hospice certification are preferred. Proficiency with EMR and BLS is expected. Join a mission-driven team delivering Full Life Care with dignity.

Qualifications

• Active Florida RN license; BSN preferred.
• Hospice certification preferred.
• Current BLS certificate.
• Ability to assess physical and psychosocial needs of patient and family.
• 1+ year in public health, home nursing, oncology, or hospice nursing preferred.

Responsibilities

• Evaluate patients referred for hospice services.
• Educate patients and families about hospice care and the continuum of care.
• Coordinate individualized hospice plans of care with physicians and liaisons.
• Liaise with hospitals, physicians, long-term care facilities, and other healthcare professionals.
• Promote hospice philosophy through education and relationship-building within the healthcare community.
