Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Care Navigator for West Palm Beach onsite in a call center. This full-time role works Thursday through Sunday, 8:00 p.m.

– 7:30 a.m., supporting admissions with an empathetic patient and family experience during life’s transitions. You will facilitate referrals to admissions, provide outstanding service to patients and referral sources, collaborate with an interdisciplinary team, and maintain accurate records in line with

• Associate degree preferred in a healthcare or related field.
• Three years of experience in healthcare administration or customer service.
• Knowledge of hospice or home health programs preferred; willingness to develop clinical understanding.

This position is **onsite** in a **call center environment.** This **full-time** position works **Thursday through Sunday, 8:00 p.m. – 7:30 a.m.**, supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.
