Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Care Navigator for West Palm Beach onsite in a call center. This full-time role works Thursday through Sunday, 8:00 p.m.

– 7:30 a.m., supporting admissions with an empathetic patient and family experience during life’s transitions. You will facilitate referrals to admissions, provide outstanding service to patients and referral sources, collaborate with an interdisciplinary team, and maintain accurate records in line with

Qualifications

Associate degree preferred in a healthcare or related field.
Three years of experience in healthcare administration or customer service.
Knowledge of hospice or home health programs preferred; willingness to develop clinical understanding.

Responsibilities

Facilitate the conversion of referrals to admissions through intake completion.
Provide exceptional customer service to patients, families, and referral sources with empathy.
Communicate effectively with interdisciplinary team members for timely admissions.
Maintain accurate records and documentation per organizational policies.
Support a compassionate, patient-centered environment reflecting our mission.
