Job summary

UnitedHealth Group’s Optum HouseCalls team is seeking a Nurse Practitioner for in-home assessments across Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible scheduling, no on-call, weekends, or holidays, and a $10,000 sign-on bonus are offered. Active NP certification and state licensure, plus a valid driver's license, are required to perform home visits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver’s license with access to reliable transportation for home visits
• If state allows NP practice independently, must obtain approval from licensing board and apply within set timelines

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history, medication reconciliation, vital signs, and physical exam
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
