Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to conduct in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible schedules (20–40 hours/week) with no on-call, weekends, or holidays requirements. A $10,000 sign-on bonus is available for external candidates and a comprehensive benefits

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state by start date.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other plan members.
• Identify diagnoses for care management and treatment planning.
• Educate members on diseases, medications, and adherence.
• Coordinate with primary care providers to address gaps in care.
• Recognize urgent situations and intervene appropriately.
