Job summary

UnitedHealth Group, through Optum HouseCalls, seeks an Advanced Practice Clinician to perform in-home health assessments for Medicare Advantage members. The role is field-based with a focus on education, gap closure, and coordination with PCPs.

The position offers a flexible schedule across 8am–8pm shifts, no on-call, weekends, or holidays. NP or PA certifications with active licensure are required, plus reliable transportation for home visits.

Qualifications

• Active ANCC or AANP certification for Family/Adult/Geriatric primary care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the state of practice.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
