Optum HouseCalls is seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

A strong emphasis on patient-centric care and accessible home visits is central to the role. Flexible scheduling within a 8am–8pm window over a 7-day week is offered, with no on-call, weekends, or holidays required.

Qualifications

• ANCC or AANP certification for NP.
• Active, unrestricted license in the practicing state.
• Active and unrestricted driver's license; access to reliable transportation.
• Ability to navigate varied home environments and perform in-home assessments.
• If applicable, eligibility with licensing board and approval timelines.

Responsabilités

• Conduct comprehensive in-home assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent/emergent situations and intervene as needed.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals.
