Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes and care coordination.

You will conduct comprehensive home visits, document diagnoses, and educate patients on disease processes and treatments, with a strong focus on accessibility and patient-centered care.

Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the practicing state and ability to obtain by start date
• Access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments including medical history and vitals
• Perform evidence-based screenings and point-of-care testing as appropriate
• Document diagnoses for care management and communicate findings to PCPs
• Educate members on diseases, medications, and adherence; address social determinants
