UnitedHealth Group’s Optum HouseCalls seeks a Nurse Practitioner to perform in‑home health assessments for Medicare Advantage members. This field‑based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You’ll work in a flexible schedule with no on‑call and no required weekends, delivering care at members’ homes across participating states. Experience with EMR and geriatric populations is preferred.

Qualifikationen

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care
• Active, unrestricted licensure in the state you will be practicing in with start date
• Driver’s license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• If applicable, ability to practice independently per state regulations

Aufgaben

• Conduct in‑home health assessments for Medicare Advantage and other plan members
• Educate patients on disease processes, medications and adherence
• Collaborate with PCPs to close care gaps and coordinate referrals
• Identify urgent/emergent situations and respond appropriately
• Document findings and support care planning during visits
