Job summary

Optum HouseCalls is seeking a skilled Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

The role offers a flexible work model, no on-call, and a comprehensive benefits package. Licensure and certifications must be current in the practicing state.

Qualifications

• Active ANCC/AANP certification for NP (Family/Adult/Geriatric/ACNP) or related specialty.
• Current NCCPA certification for PA and state licensure.
• Unrestricted licensure in practicing state.
• Driver's license and reliable transportation for home visits.

Responsibilities

• Perform in-home assessments for Medicare Advantage and other members.
• Document diagnoses and care planning based on assessments.
• Communicate findings to primary care providers to close care gaps.
• Identify urgent situations and intervene appropriately.
