Job summary

Optum is actively seeking a HouseCalls Advanced Practice Clinician to perform in-home health assessments for our Medicare Advantage members. In this role, you will work flexible hours between 8 am and 8 pm with no on-call days or weekends.

This innovative role focuses on improving health outcomes through education and collaboration, allowing you to work directly within patients' homes. Compensation ranges from $109,500 to $164,000, alongside a comprehensive benefits package.

Qualifications

• Must have a certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Must maintain an active, unrestricted licensure for practicing.
• Ability to perform assessments and navigate diverse home settings.

Responsibilities

• Conduct in-home health assessments annually.
• Identify and communicate diagnoses for treatment planning.
• Educate members on medications and disease processes.
