Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. You will deliver education, gap-closure, and care coordination with primary care providers in patients’ homes.

This field-based role offers flexible scheduling, 24–40 hours per week, and no on-call, weekends, or holidays. Opportunities for growth and collaboration within an interdisciplinary team are emphasized.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (for NP) or NCCPA certification (for PA).
• State licensure in the practicing state without disciplinary actions.
• Driver’s license with reliable transportation for home visits.

Responsibilities

• Perform comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and planning; coordinate with PCPs to close gaps in care.
• Educate patients on disease processes, medications, and adherence; address social determinants of health and provide referrals.

Education
