Job summary

UnitedHealth Group's Optum HouseCalls is seeking a licensed NP or PA to perform in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve outcomes.

Enjoy flexible scheduling (8am-8pm, across 7 days) with no on-call, weekends, or holidays. Competitive compensation plus benefits and a $10,000 sign-on bonus for external candidates may apply.

Qualifications

• Active ANCC or AANP certification in relevant family care tracks.
• NCCPA certification (PA) and state licensure required.
• Unrestricted licensure in practicing state and valid driver's license.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct in-home assessments with medical history review, vitals, and exams.
• Document diagnoses for care management and planning.
• Communicate findings with patients' PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate patients on diseases, meds, and adherence.
• Address social determinants and provide referrals as needed.
