Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based, non-prescribing role focuses on education, care-gap closure, and collaboration with PCPs.

Flexible scheduling in an 8am–8pm window across a 7-day week, with no on-call, weekends, or holidays. Competitive total cash compensation, benefits, and growth opportunities within the Optum family.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Ability to navigate varied home environments and perform assessments.
• New hires eligible but must apply for independent practice where allowed within timelines.

Responsibilities

• Conduct in-home assessments including medical history review, vitals, and physical exam.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
