Enhabit Home Health & Hospice is seeking a scheduling coordinator to efficiently assign patients to field clinicians and ensure seamless care coordination. The role requires strong communication, licensing as an LPN/LVN, and at least one year of clinical healthcare experience.

The position emphasizes on-site attendance with a need for reliable transportation and proficiency in MS Office and mobile devices. CPR certification is required, with home health experience preferred.

Formación

• High school diploma or equivalent required; LPN/LVN license required; valid clinical license per state rules.
• Minimum one year of clinical experience in healthcare.
• CPR certification required.
• One year of home health, hospice, or pediatric experience in the last 24 months preferred with scheduling experience.
• Valid driver license and automobile liability insurance.
• Basic technology skills, including MS Office.

Responsabilidades

• Schedule patients to field clinicians.
• Coordinate communication among patients, field staff, physicians, referral sources, and caregivers.
• Maintain the on-call notebook and hospitalization logs to improve communication among stakeholders.
