Job summary

Enhabit Home Health & Hospice is seeking a Nurse Scheduler/Coordinator to arrange patient visits with field clinicians and ensure seamless care coordination. You will communicate with staff, patients, physicians, and families to sustain continuity of care and manage on-call notes and hospital logs.

The role requires an LPN/LVN license, CPR certification, and a valid driver license, with on-site attendance essential to support supervision and teamwork.

Qualifications

• Must have High School Diploma or equivalent.
• Licensed practical or vocational nurse required.
• CPR certification required.
• Valid state driver license in current state of residence.
• Automobile liability insurance as required by law.
• Demonstrated technology skills including operation of a mobile device.

Responsibilities

• Scheduling patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
