Job summary

Enhabit Home Health & Hospice is seeking a dependable scheduling coordinator to align patients with field clinicians, ensuring timely care and clear communication among patients, physicians, referral sources, and caregivers.

This on-site role requires a High School Diploma or equivalent, a valid LPN/LVN license, CPR certification, a valid driver license, and solid Microsoft Office skills; bilingual English/Spanish is a plus. Join a collaborative team delivering compassionate home-based care.

Qualifications

• High School Diploma or equivalent is required.
• LPN/LVN license is required per state rules.
• Minimum of one year of clinical experience in a healthcare setting.
• CPR certification is required.
• Must have and maintain a valid state driver license and automobile liability insurance.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance stakeholder communication.
