Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to arrange patient visits with field clinicians and support seamless care coordination. You will liaise with patients, physicians, caregivers, and staff to ensure timely, accurate scheduling and documentation.

The role requires an LPN/LVN license, CPR, and at least one year of clinical experience. Strong communication, digital skills, and reliability are essential for success in this on-site position.

Qualifications

• High School Diploma or equivalent is required.
• Licensed practical or vocational nurse with current state license is required.
• CPR certification is required.
• Minimum of one year of clinical healthcare experience is required.
• Valid state driver license and automobile liability insurance are required.
• Basic demonstrated technology skills including use of a mobile device are required.
• Experience with Homecare Homebase (HCHB) is preferred.
• English fluency; bilingual English/Spanish is preferred.
• In-office attendance is essential for supervision and teamwork.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other providers to maintain care coordination.
• Manage on-call notebook and hospitalization logs to enhance communication among stakeholders.
